‌Annual Immunization Assessment Reporting for 2025-2026

Each year, California law requires schools to review and report student immunization records for children in pre-kindergarten/child care (ages 2–5), kindergarten, and 7th grade. Canterbury’s office compiles this information in accordance with the California Department of Public Health (CDPH) requirements and submits the report by the state deadline of December 5, 2025.

Submitting Your Child’s Records

Parents may submit updated immunization records in two ways:

  • Upload directly into your FACTS Family Portal.
  • Submit a paper copy of your child’s record to the school office.

Please make sure your child’s record is complete, current, and clearly legible.

Summary of Requirements

  • Child Care / Pre-Kindergarten (ages 2–5): documentation of required doses for DTaP, Polio, Hib, Hepatitis B, MMR, and Varicella.
  • Transitional Kindergarten / Kindergarten: documentation of required doses for DTaP, Polio, Hepatitis B, MMR, and Varicella.
  • 7th Grade: documentation of Tdap booster and two doses of Varicella.

Student records are protected by state and federal privacy laws. Canterbury respects the authority of parents in all medical decisions: no medication or vaccine will ever be administered on our campus without explicit and direct parental consent, nor will state agents be permitted to administer treatments to students for any reason.

Notes from the State Health Dept Below:

California Health and Safety Code Section 120375 and California Code of Regulations section 6075 require all child care facilities and schools (including home schools) to review student immunization records and report annually on the immunization status of their children in pre-kindergarten/child care (ages 2-5 years), TK-kindergarten, or 7th grade.

Medical Exemptions

In 2021, CDPH launched the California Immunization Registry – Medical Exemption (CAIR-ME) website as the only method in California to issue and track new medical exemptions from required vaccines for children attending school or child care facilities. Once you receive a new medical exemption from an incoming enrollee, please request access to CAIR-ME by emailing medicalexemptions@cdph.ca.gov. You will be able to view and verify the status of medical exemptions in CAIR-ME for children attending your school or facility. SB 277 Exceptions

Under Senate Bill 277, immunization requirements do not apply to students who attend a home-based private school or an independent study program and do not receive classroom-based instruction. Students who have an individualized education program (IEP) should continue to receive all necessary services identified in their IEP regardless of their vaccination status.

Reporting will continue to monitor all students who are missing vaccines for any reason, including reasons allowed by Senate Bill 277. Schools are still required to request, maintain, and report records of immunizations that have been received for these students.
